Blue Dress Walk — Liability Release & Participation Waiver A Fun Walk for Colon Cancer Awareness By participating in the Blue Dress Walk, I acknowledge that this event involves physical activity and outdoor conditions that may include uneven surfaces, weather exposure, crowds, traffic, and other inherent risks. Assumption of Risk I voluntarily choose to participate and assume all risks associated with my involvement in the Blue Dress Walk, whether known or unknown, including the risk of personal injury, illness, property damage, or other loss. Health Acknowledgment I certify that I am physically able to participate in this activity and have not been advised otherwise by a medical professional. I understand that I am responsible for my own safety, hydration, and well-being during the event. Release of Liability In consideration of being allowed to participate, I hereby release, waive, and hold harmless Blue Dress Walk, Memorial Hospital Foundation, Chandeleur Brewing, event sponsors, volunteers, and their respective officers, directors, employees, partners, and agents from any and all claims, demands, damages, or causes of action arising out of or related to my participation in the event, except where prohibited by law. Photo & Media Consent I grant permission for Blue Dress Walk and its partners to photograph, film, or record me during the event and to use my likeness in event recaps, promotional materials, social media, and future marketing without compensation. Minor Participants If registering a participant under the age of 18, I certify that I am the parent or legal guardian and agree to this waiver on their behalf. By checking the consent box or signing electronically in the registration form, I confirm that I have read, understand, and agree to this liability release and waiver.
